Our Publication Scheme
This Publication Scheme is a guide to the information routinely published by Northgate Dental (UI Associates Limited). It describes the classes and types of information about our practice and team that we make publicly available.
By adopting this scheme, we are committed to making the information listed in Part Two available to members of the public. The scheme is reviewed regularly and monitored to ensure it meets legal requirements and patient needs. We aim to make this information easy to find and accessible.
Your Rights
In addition to the information in this scheme, you are entitled to request other information about Northgate Dental from the Practice Manager, Umar Ilyas. We may not always be able to provide some or all of the information you request. If this happens, we will explain why.
Under the Data Protection Act 2018 and the UK General Data Protection Regulation (GDPR), you are entitled to access your clinical records or any personal information held about you by our practice. Please contact the Practice Manager, Umar Ilyas, if you would like a copy of your records.

If you have a complaint about how we have handled your request for information, please contact the Practice Manager, Umar Ilyas. If you remain dissatisfied, you may escalate your complaint to the relevant external bodies (see Part Two).
